If you are experiencing a dental, medical, or any other emergency, do not use this website’s contact form. Call 911 or go to the nearest emergency room immediately. We cannot monitor the website contact form in real time and cannot respond to emergency situations through this channel.
Appointment Requests
Submitting an appointment request form does not confirm or guarantee an appointment. After we receive your request, a member of our team will contact you to discuss availability and confirm a date and time. An appointment is confirmed only when we have both agreed on a specific time.
